En los pacientes con histiocitosis de células de Langerhans diseminada con compromiso multiorgánico, la terapia de mantenimiento con interferón-α, después de un curso inicial con etopósido, parece ser efectiva para prevenir las recurrencias.

Resumen
El tratamiento de los pacientes que padecen una forma diseminada de la histiocitosis de células de Langerhans (HCL) todavía es controvertido. Hasta ahora, se han realizado pocos estudios amplios y aleatorizados. Presentamos 3 pacientes con una forma diseminada de HCL, de 4 y 9 meses y 2 años, respectivamente. Las células de Langerhans afectadas mostraron reacción inmunohistoquímica positiva a la proteína S-100, y mediante microscopia electrónica se confirmó la presencia de gránulos de Birbeck. Todos los pacientes se trataron con etopósido (VP-16), a 200 mg/m2 durante 3 días consecutivos, con 15 ciclos a intervalos de 3 semanas entre cada uno, seguidos por una terapia de mantenimiento con IFN-α. Todos alcanzaron una remisión estable completa.Nuestros enfermos fueron muy jóvenes, de alto riesgo, con compromiso multiorgánico, y dos de ellos con signos obvios de disfunción orgánica al momento de presentación, sugerentes de mal pronóstico. Todos permanecieron libres de enfermedad por varios años después del tratamiento. Concluimos que el INF-α puede prevenir las recurrencias en pacientes de alto riesgo.

Abstract
The treatment of patients who suffer from a disseminated form of Langerhans cell histiocytosis (LCH) is still controversial. So far, few larger randomized studies have been performed. We present three patients with a disseminated form of LCH, 4 months, 9 months, 2 years old, respectively. The lesional Langerhans cells in each patient showed positive immunohistochemical reaction to S-100 protein and the presence of Birbeck granules was confirmed by electron microscopy. All the patients were treated with etoposide (VP-16), 200 mg/m2 three consecutive days, with fifteen cycles at intervals of three weeks between each cycle, followed by maintenance therapy with IFN-a. All three patients reached complete stabile remission. Our patients were young, at high risk, with multiple organ involvement of LCH, and two of them had obvious signs of organ dysfunction at presentation suggesting a poor prognosis. All remain disease-free several years after therapy. We conclude that INF- may prevent recurrences in high-risk patients.
