La angioplastia con implante de stent para la estenosis grave del ostium de la arteria renal mejora la función del órgano y el control de la presión arterial en un espectro amplio de pacientes.

Resumen
Estudiamos 215 pacientes consecutivos con estenosis aterosclerótica del ostium de la arteria renal (EAR), con un diámetro de dicha estenosis mayor o igual al 70%, sometidos a angioplastia con colocación de stent. Para la realización del análisis, 191 casos se hallaban con vida al año de efectuado el procedimiento. En el 52% de los pacientes (89/191) el valor medio de la concentración plasmática de creatinina descendió en forma significativa, desde 1.21 mg/dl (cuartilos: 0.92; 1.60 mg/dl) al inicio hasta 1.10 mg/dl (cuartilos: 0.88; 1.50 mg/dl) al año (p = 0.047). Los predictores independientes y significativos de mejoría de la función renal fueron el valor inicial de creatinina plasmática, odds ratio [OR] 2.58; intervalo de confianza del 95% [IC95] 1.35-4.94; p = 0.004) y la función ventricular izquierda (1.51 [1.04-1.21]; p = 0.032). En promedio, la presión arterial media descendió significativamente desde 102 ± 12 mm Hg (media ± desvío estándar [DE) hasta 92 ± 10 mm Hg al año (p < 0.001). El sexo femenino, el valor inicial elevado de presión arterial y el grosor normal del parénquima renal fueron predictores independientes del descenso de la presión arterial. Estos resultados apoyan el ofrecimiento de la angioplastía con colocación de stent para la EAR originada en el ostium y de causa aterosclerótica a un espectro de pacientes más amplio de lo que se pensaba con anterioridad.

Abstract
We studied 215 consecutive patients with atherosclerotic ostial renal artery stenosis (RAS) of ≥ 70% diameter stenosis undergoing stent-supported angioplasty with 191 surviving patients available for 1 year follow-up examination. In 52% of the patients (99/191) mean serum creatinine concentration decreased significantly from 1.21 mg/dl (quartiles: 0.92; 1.60 mg/dl) at baseline to 1.10 mg/dl (quartiles: 0.88; 1.50 mg/dl) at 1 year (p = 0.047). Significant independent predictors of improved renal function were baseline serum creatinine (odds-ratio [95% confidence interval]: 2.58 [1.35 to 4.94], p = 0.004) and left ventricular function (OR 1.51 [1.04-2.21], p = 0.032). On average, mean arterial blood pressure decreased significantly from 102 ± 12 mm Hg (mean ± SD) at baseline to 92 ± 10 mm Hg at 1 year (p < 0.001). Female gender, high baseline mean blood pressure and normal renal parenchymal thickness were independent predictors for decreased mean blood pressure. These results support to offer stent-supported angioplasty for atherosclerotic ostial RAS to a broader spectrum of patients than previously thought.
