MODIFICACION DE LA PRESCRIPCION DE ANTIPSICOTICOS ANTE UNA INTERVENCION DESTINADA A MEJORAR EL DESEMPEÑO DE LOS PSIQUIATRAS

Resumen
En noviembre de 2001 comenzó una iniciativa de intensidad moderada para lograr mejorar el desempeño de los profesionales en un hospital psiquiátrico estatal con el objetivo de disminuir la politerapia antipsicótica. El Jefe de Psiquiatría se reunió con cada uno de los 14 psiquiatras, comparó los índices de politerapia entre los colegas y solicitó una disminución del 10% de la politerapia antipsicótica. La politerapia disminuyó significativamente, ya que se observó en el 40% de los pacientes tratados con antipsicóticos en noviembre de 2001 y en el 31% en agosto de 2002. La disminución no se relacionó con los índices iniciales de politerapia o con el aumento del uso de otros psicofármacos. Sin embargo, los índices de politerapia retornaron al 40% entre 2006 y 2008. Durante un período de cambio institucional posiblemente asociado con un aumento de la cantidad de pacientes agudos, la politerapia alcanzó el 67%. Si bien la intervención de intensidad moderada efectuada por un Jefe de Servicio en 2001 redujo la politerapia, son necesarios estudios adicionales para determinar si dicha estrategia puede ser eficaz en presencia de poblaciones de pacientes con cuadros clínicos más agudos.

Abstract
A moderate intensity performance improvement initiative at a state psychiatric hospital to reduce antipsychotic polypharmacy was undertaken in November, 2001. The Chief of Psychiatry met with each of 14 psychiatrists, comparing their rates of polypharmacy with that of their peers, and asked all to attempt a 10% decrease in antipsychotic polypharmacy. Polypharmacy fell significantly from 40 percent of patients treated with antipsychotics in November, 2001 to 31 percent in August 2002. Reduced polypharmacy was not related to baseline rates or associated with increased use of other psychotropic medications. Rates of polypharmacy in 2006-2008 appear to have reverted to the 40% range, however. During a period of institutional change, possibly associated with increased clinical acuity, antipsychotic polypharmacy approached 67%. While an only modestly intensive intervention by leadership appeared to reduce polypharmacy in 2001, studies are required to determine whether such efforts can be efficacious under different, more acute clinical conditions.
