EFECTO DE LA HIGIENE BUCAL, EL PH SALIVAL Y EL TABAQUISMO EN PACIENTES CON ENFERMEDAD PERIODONTAL EN UNA CLINICA ESPECIALIZADA EN CONTROL DE ADICCIONES

La prevalencia de enfermedad periodontal fue 100%, se asoció su gravedad con el sexo masculino (p = 0.003), mayor edad (p = 0.001), deficiente higiene bucal, pH alcalino, mayor tiempo con tabaquismo, mayor número de cigarrillos fumados al día y cigarrillos consumidos durante toda la vida (p = 0.000 cada una). Los factores de riesgo estudiados representan juntos un mayor riesgo para enfermedad periodontal.

Resumen
Introducción: La enfermedad periodontal (EP) es causada principalmente por la presencia de bacterias periodontopatógenas concentradas en la biopelícula, el sarro contribuye a su retención. Se ha informado asociación entre desarrollo y progresión de periodontitis y tabaquismo. El propósito del trabajo fue determinar prevalencia de EP y su asociación con higiene bucal, pH salival y tabaquismo en pacientes en tratamiento para control de adicciones. Materiales y métodos: Estudio cuantitativo, descriptivo, observacional y transversal. Se solicitó consentimiento informado, se aplicó cuestionario estructurado para determinar tabaquismo. Bajo medidas para control de infecciones se evaluó la calidad de higiene bucal (IHOS), nivel de salud periodontal (IPC), pH salival (tiras reactivas Hydrion). Se determinaron medidas de tendencia central, dispersión y asociación entre variables (Stata SE/11.1). Resultados: La población estudiada estuvo constituida por 45 individuos, prevaleció el sexo masculino, intervalo de edad 16 a 55 años, mediana 25 años (Q1 = 21; Q3 = 34). La calidad de higiene bucal fue regular en 53.3%. La prevalencia de EP fue 100%, el tabaquismo positivo en 86.6%. La saliva fue alcalina en 31 pacientes (68.9%) y en 14 (31.1%), neutra. Se asoció la gravedad de la EP con sexo masculino (p = 0.003), mayor edad (p = 0.001), deficiente higiene bucal (p = 0.000), pH alcalino (p = 0.000), mayor tiempo de uso del tabaco (p = 0.000), mayor número de cigarrillos al día (p = 0.000) y cigarrillos consumidos durante toda la vida (p = 0.000). Se asoció pH alcalino con sexo masculino (p = 0.035), higiene bucal deficiente (p = 0.001) y tabaquismo, (p = 0.000). Conclusiones: Los resultados sugieren que la higiene bucal, pH salival y tabaquismo son factores que potencializan la severidad de la EP.

Abstract
Introduction: Periodontal disease (PD) is caused mainly by the presence of periodontal pathogenic bacteria concentrated in the biofilm; calculus contributes to its retention. An association between development and progression of periodontitis and smoking has been reported. The purpose of the study was to determine the prevalence of PD and its association with oral hygiene, salivary pH and smoking in patients under treatment for addictions control. Materials and methods: Quantitative, descriptive, observational and transversal study. Informed consent was requested. For determining smoking status a structured questionnaire was applied. Under measures for infection control, quality of oral hygiene (IHOS), periodontal health level (PCI) and salivary pH (Hydrion test strips) were evaluated. Measures of central tendency, dispersion and association between variables were determined (Stata SE/11.1). Results: The studied population consisted of 45 individuals, the male sex prevailed, age range 16-55 years, median 25 years (Q1 = 21; Q3 = 34). Oral hygiene quality was regular in 53.3%. The prevalence of PD was 100%, positive smoking was 86.6%. Alkaline saliva in 31 patients (68.9%), in 14 (31.1%) neutral. Severity of PD was associated with male sex (p = 0.003), older age (p = 0.001), poor oral hygiene (p = 0.000), alkaline pH (p = 0.000), longer time of tobacco use (p = 0.000), greater number of cigarettes per day (p = 0.000) and cigarettes consumed throughout life (p = 0.000). Alkaline pH was associated with male sex (p = 0.035), poor oral hygiene (p = 0.001) and smoking (p = 0.000). Conclusions: The results suggest that oral hygiene, salivary pH and smoking are factors that potentiate the severity of PD.
