En la actualidad, ninguna terapia presenta una tasa de erradicación del 100% de la infección por Helicobacter pylori. Un nuevo tratamiento secuencial de 10 días alcanzó una tasa de erradicación de 97% en niños. Los probióticos pueden ser útiles, al aumentar la adhesión de los pacientes mediante la disminución de los efectos secundarios de los antibióticos.

Resumen
Aproximadamente el 50% de la población mundial está infectada por Helicobacter pylori, con la mayor tasa de prevalencia en los países en desarrollo. La infección causa gastritis y úlcera péptica y está asociada con la aparición de cáncer gástrico. Además, adquirir la infección a temprana edad sugiere un incremento del riesgo de padecer cáncer a lo largo de la vida. Varios regímenes de tratamiento se han propuesto para curar esta infección, pero ninguna terapia alcanza el 100% de la tasa de erradicación en el primer ensayo. Las normativas actuales sugieren el uso de la terapia triple como primera elección en el tratamiento de la infección por H. pylori. Desafortunadamente, en los últimos años estos esquemas mostraron una progresiva reducción de la eficacia con tasas de erradicación menores del 75%. Actualmente, la resistencia bacteriana y los efectos adversos representan la causa más frecuente de fracaso del tratamiento anti-H. pylori en la práctica clínica. Con el objeto de mejorar el resultado de la tasa de erradicación, indicamos, en un ensayo clínico aleatorizado, un nuevo tratamiento secuencial de 10 días y obtuvimos una tasa de erradicación de 97%. El uso de probióticos fue propuesto recientemente en adultos y niños para aumentar la tolerancia de los pacientes al limitar los efectos colaterales de las terapias de erradicación de H. pylori. Finalmente, la nueva generación de fluoroquinolonas, como parte de los regímenes de erradicación, parece promisoria.

Abstract
Approximately 50% of the world population is infected with Helicobacter pylori, with the highest prevalence rates in developing countries. The infection causes gastritis and peptic ulcers and is associated with the development of gastric cancer. Furthermore, younger age at acquisition has been suggested as a means to increase the risk of developing cancer later in life. Several treatment regimens have been proposed to cure this infection, but no therapy leads to a 100% eradication rate at first attempt. The current guidelines suggest the use of triple therapy as first choice treatment of H. pylori infection. Unfortunately, in recent years these schedules have shown a progressive reduction in efficacy with eradication rates below 75%. Today, bacterial resistance and side effect occurrence represent the most frequent cause for anti-H. pylori treatment failure in clinical practice. In an attempt to improve eradication success rates, we used in a randomised clinical trial a novel 10 day sequential treatment and achieved an eradication rate of 97%. To increase patients' tolerability by limiting side effects of anti-H. pylori eradicating therapies, the use of probiotics has recently been proposed in adults and children. Finally, the newer generation fluoroquinolones have shown some promise as part of an eradication regimen.
