Abstract
Incus transposition remains a useful technique for ossicular chain reconstruction. I have previously shown that the early post-operative improvements in the air-bone gap (ABG) after incus transposition can be maintained with long term follow-up (mean follow-up 9 years). I present a review of the literature on incus transposition since the publication of the above series and look forward to what the future of ossiculoplasty may hold particularly with reference to titanium ossiculoplasty, comment on its possible shortcomings and look at the increasing popularity of bone anchored hearing aids.

Key words
incus, tympanoplasty, middle ear, titanium
