EL VERAPAMILO ES EL AGENTE TERAPEUTICO OPTIMO PARA LA ENFERMEDAD DE PEYRONIE

El verapamilo, aplicado como inyecciones intraplaca o en forma tópica ayudado por corriente eléctrica (EMDA), es por el momento el tratamiento no quirúrgico de elección en la enfermedad de Peyronie, ya que logra reducir el proceso de cicatrización y disminuir la curvatura del pene.

Resumen
Al ser un modificador biológico de la matriz extracelular de fibroblastos y citoquinas involucrados en el proceso de cicatrización, el verapamilo, un bloqueante de los canales de calcio, surgió como la droga más renombrada a nivel científico para tratar la enfermedad de Peyronie. Si bien no se demostró que ningún agente oral o inyectable tenga éxito uniforme en reducir las deformaciones que se observan en el pene de los hombres con enfermedad de Peyronie, se informó en varias oportunidades que el verapamilo disminuye la curvatura en hasta un 60% en los hombres que completan un curso de tratamiento de 12 inyecciones. Además, parece ser una terapia segura, ya que son pocos los que informan empeoramiento de la deformidad, y prácticamente no tiene eventos adversos graves. Los hombres que no respondan a este tratamiento pueden someterse a cirugía, sin que haya ninguna dificultad agregada. Por lo tanto, parece ser un tratamiento seguro y razonablemente efectivo, que produce disminución de la deformidad, más allá de su gravedad o de la duración de la enfermedad. Actualmente está siendo investigado el uso tópico de verapamilo, aplicado solo o con fuentes de energía. Es claro que se requiere investigar más estos enfoques menos invasivos. En este artículo se revisa la base científica del uso de verapamilo, las controversias acerca de su utilización, la técnica de inyección, y la experiencia personal en los últimos 12 años en 800 hombres con enfermedad de Peyronie.

Abstract
As a biologic modifier of extra-cellular matrix fibroblasts and cytokines involved in the wound healing process, verapamil, a calcium-channel blocker, has emerged as the most scientifically sound drug for the treatment of Peyronie's disease. Although no oral or injectable agent has been demonstrated to be uniformly successful in reducing the deformities seen in the penis of men with Peyronie's disease, verapamil has consistently been reported to result in diminished curvature in up to 60% of men completing a course of 12 injection treatments. In addition it appears to be a safe therapy with few men reporting worsening of deformity and virtually no serious adverse events. Men who fail this treatment may proceed to surgery without added difficulty. Therefore it appears to be a safe and reasonably effective therapy which has been noted to result in improvement in deformity regardless of the duration of disease or severity of deformity. Topical use of verapamil applied with and without energy sources are now being reported.
Clearly further investigation is necessary for these less invasive approaches. This article reviews the scientific basis for using verapamil, the controversies around its use, a discussion of injection technique and personal experience over the last 12 years with over 800 men with Peyronie's disease.
