ANALISIS DE INTERVALOS ELECTROCARDIOGRAFICOS EN NIÑOS CON INSUFICIENCIA RENAL CRONICA

La información sobre pacientes tanto adultos como pediátricos receptores de trasplante, con insuficiencia renal crónica y enfermedad renal terminal apuntan hacia un mayor riesgo de mortalidad cardíaca, de la cual una proporción significativa es debida a muerte súbita y arritmias cardíacas. Estos pacientes tienen una prevalencia en la prolongación del intervalo QT corregido, aunque la causa exacta de esta alteración todavía se desconoce.

Resumen
Un porcentaje elevado de la mortalidad encontrada en niños con insuficiencia renal y posterior al transplante renal responde a causas cardíacas. Dentro de esta categoría, una proporción importante de las muertes se atribuye a disritmias cardíacas que llevan a muerte súbita. La literatura sobre nefrología adulta demuestra claramente alta prevalencia de la prolongación del QTc e incrementada dispersión del QTc en la población con insuficiencia renal, en comparación con controles sanos. Los datos sobre niños son mucho más limitados. El propósito de este informe es rever críticamente la literatura pediátrica, y cuando sea necesario también la adulta, para determinar la prevalencia de diversas anormalidades en los intervalos electrocardiográficos en pacientes con insuficiencia renal, aquellos en diálisis y aquellos que han recibido un transplante renal. La asociación de estas anormalidades electrocardiográficas con variables demográficas y otras serán discutidas, así como las consecuencias clínicas de estas anormalidades. Por último, basándose en los datos analizados, se formularán recomendaciones para el monitoreo cardíaco de estos pacientes.

Abstract
Cardiac deaths account for a large percentage of the mortality encountered in children with renal failure and post renal transplantation. Within this category, a significant proportion of deaths are attributed to cardiac dysrhythmias leading to sudden death. The adult nephrology literature clearly demonstrates a higher prevalence of QTc prolongation and increased QTc dispersion in the renal failure population, compared to healthy controls. Data in children are much more limited. The purpose of this report is to critically review the pediatric and, where pertinent, the adult literature to determine the prevalence of various electrocardiographic interval abnormalities in patients with renal failure, those on dialysis, and those who have received renal transplants. The association of these electrocardiographic abnormalities with demographic and other variables will be discussed, as will the clinical implications of these abnormalities. Lastly, based on the data reviewed, recommendations on cardiac monitoring in this population will be provided.
