ADULTOS CON MENINGITIS CAUSADA POR ESTREPTOCOCOS VIRIDANS: CARACTERISTICAS CLINICAS, ESPECIES DE ESTREPTOCOCOS VIRIDANS Y RESULTADOS TERAPEUTICOS

El 6% de las meningitis de los adultos corresponde a meningitis por distintas especies de estreptococos viridans, en su mayoría adquiridas en forma extrahospitalaria.

Resumen
Diecinueve pacientes adultos con meningitis causada por estreptococos viridans fueron enrolados en este estudio, a partir del análisis de los datos clínicos recolectados durante 17 años. Las meningitis causadas por estreptococos viridans representaron el 6% (19/294) de todos los pacientes adultos ingresados con meningitis bacteriana comprobada por cultivo en ese período. Ochenta y cuatro por ciento (16/19) habían contraído la infección en forma extrahospitalaria y 16% (3/19), luego de neurocirugía. Todas las cepas fueron susceptibles a penicilina. La puerta de entrada de la infección pudo determinarse en 18 pacientes, no así en el restante. De los 18 pacientes con puerta de entrada conocida, 7 fueron infecciones otofaríngeas con diseminación hemática o sin ella; 7, septicemias; 1, septicemia con endocarditis;1, postraumática con diseminación hemática y endocarditis; 1, postraumática con diseminación hemática; y 1, poscraneotomía. Sólo hubo un fallecimiento en estos 19 casos. Las manifestaciones clínicas de meningitis bacteriana del adulto causada por estreptococos viridans son variables, y nuestros pacientes tenían en su mayoría enfermedades subyacentes. Este estudio también demuestra la alta incidencia de supuración focal y vasculitis cerebral concomitante con la meningitis. La penicilina sigue siendo el antibiótico de elección en el tratamiento de las meningitis causadas por estreptococos viridans. La prognosis es favorable si el diagnóstico es precoz y el tratamiento, temprano.

Abstract
Nineteen adult patients with meningitis caused by viridans streptococci, were enrolled for this study, with clinical data collected over a period of 17 years used for analysis. Meningitis caused by viridans streptococci accounted for 6 % (19/294) of all enrolled patients culture-proven for adult bacterial meningitis. Eighty-four percent (16/19) of the patients contracted the infection through the community, and 16% (3/19) contracted the disease postneurosurgery. All the isolates were susceptible to penicillin. The portal-of-entry of infection was determined in 18 patients and unknown in one. Of the 18 patients with known portal-of-entry of infection, 7 were otopharyngeal infection with or without hematogenous spread, 7 were hematogenous spread, one was hematogenous spread with endocarditis, one was posttraumatic state and hematogenous spread with endocarditis, one was posttraumatic state and hematogenous spread and one was postcraniotomy. Only one fatality occurred in these 19 cases. The clinical manifestations of adult bacterial meningitis caused by viridans streptococci are variable, and most of our patients had underlying conditions. This study also demonstrates the high incidence of focal suppuration and cerebral vasculitis concomitant with meningitis. Penicillin remains the choice of antibiotic in the treatment of meningitis caused by viridans streptococci. Prognosis is favorable with early diagnosis and prompt treatment.
