LA SALUD PERIODONTAL DE LOS PACIENTES CUMPLIDORES REGULARES E IRREGULARES DURANTE UNA TERAPIA DE MANTENIMIENTO

El presente estudio mostró que los pacientes cumplidores regulares presentaron mejores indicadores de salud periodontal, que incluyeron profundidad al sondaje, nivel de inserción clínica, sangrado al sondaje y niveles de placa bacteriana, en comparación con los pacientes cumplidores irregulares. Estos resultados destacan la influencia del cumplimiento de las citas de mantenimiento en la estabilidad de los tejidos periodontales.

Resumen
Objetivo: Este estudio prospectivo comparó el comportamiento de la salud periodontal en pacientes cumplidores regulares e irregulares que participaron en un programa de mantenimiento durante un año. Metodología: Durante el seguimiento participaron 54 pacientes diagnosticados inicialmente con periodontitis, quienes al terminar su tratamiento periodontal fueron incluidos en un programa de mantenimiento. Ellos se dividieron en dos grupos: 32 cumplidores regulares (CR) que no perdieron ninguna cita de mantenimiento y 22 cumplidores irregulares (CI) que perdieron al menos una cita de mantenimiento, programada trimestralmente, durante el transcurso de un año. En las cuatro citas de mantenimiento, agendadas para cada paciente, se evaluaron los siguientes indicadores de salud periodontal: nivel de inserción clínica (NIC), profundidad al sondaje periodontal, sangrado al sondaje y niveles de placa bacteriana. Resultados: Al comparar los dos grupos, se observó un mayor promedio de pérdida de NIC en los pacientes CI, comparado con los CR, aunque sin diferencias estadísticamente significativas; sin embargo, en los otros tres indicadores periodontales sí se observaron diferencias estadísticamente significativas entre los dos grupos, observándose promedios más saludables en los pacientes CR. Por otra parte, se encontraron diferencias estadísticamente significativas en todos los indicadores de salud periodontal, al comparar el estado periodontal entre la primera y cuarta cita de mantenimiento, al interior de cada grupo. Conclusiones: Después de un año de evaluación, los pacientes CR presentaron mejores indicadores de salud periodontal, comparado con los pacientes CI. Estos resultados destacan la influencia del cumplimiento de las citas de mantenimiento en la estabilidad de los tejidos periodontales.

Abstract
Objective: this prospective study compared the behavior of periodontal health, in regular and irregular compliant patients who participated in a maintenance program for one year. Methodology: the sample consisted of 54 patients initially diagnosed with periodontitis, who were included in a maintenance program at the end of their periodontal treatment. They were divided into two groups: 32 regular compliers (RC) who did not miss any maintenance appointments and 22 irregular achievers (IC) who missed at least one maintenance appointment, scheduled quarterly, over the course of a year. In the four maintenance appointments, scheduled for each patient, the following indicators of periodontal health were evaluated: clinical attachment level (CAL), periodontal probing, bleeding on probing and levels of bacterial plaque. Results: when comparing the two groups, a higher mean CAL loss was observed in the IC patients, compared to the RC, although without statistically significant differences; however, in the other three periodontal indicators if statistically significant differences were observed between the two groups, more healthy averages were observed in RC patients. On the other hand, statistically significant differences were found in all indicators of periodontal health, when comparing the periodontal status between the first and fourth maintenance appointments, within each group. Conclusions: after one year of evaluation, RC patients presented better indicators of periodontal health, compared with IC patients. These results highlight the influence of compliance with maintenance appointments on the stability of periodontal tissues.
