Nuestro objetivo es implantar un sistema de vigilancia continuo en unidades quirúrgicas. La herramienta diseñada, identifica puntos débiles del sistema, analiza causas, mide efectos, es fuente de aprendizaje de los errores y mejora la cultura de seguridad de los profesionales.

Resumen
Las acciones ligadas a la atención sanitaria conllevan un riesgo potencial para el paciente, tanto más cuanto más compleja sea dicha actuación. El ámbito quirúrgico es sin duda una de las áreas asistenciales más complejas y el entorno propicio para la existencia de riesgos para el paciente. Los sistemas de registro de eventos adversos son un instrumento de evaluación de la calidad asistencial y permiten determinar factores asociados para implementar acciones preventivas. Nuestra experiencia en un servicio de cirugía general de un hospital universitario consistió en crear una herramienta de registro basado en la vigilancia continua de los eventos del paciente. El objetivo de este trabajo es describir la evolución que ha sufrido nuestra herramienta para ajustarse a sus dos propósitos: registrar información para el aprendizaje y mejora de la seguridad del paciente, y responsabilizar a los profesionales de la gestión de riesgos y errores, como una tarea más de su actividad diaria. La versión actualizada que presentamos es un programa informático adjunto al sistema de Prescripción Electrónica de la Intranet del hospital, específico del Servicio de Cirugía. Como nueva aportación facilitadora del uso del registro, el diseño es más visual, lo que permite una percepción rápida de las complicaciones, con un código de colores de los diferentes eventos que se presentan en cada paciente, lo que brinda a los cirujanos la máxima utilidad para conocer el estado de los sujetos hospitalizados.

Abstract
Every action taken in healthcare has a potential risk for the patient; the more complex the action becomes the higher the risk. The surgical area is, without doubt, one of the areas where more complex situations emerge, and it is the environment where those risks may arise. Adverse event logging systems are quality of care evaluation tools that allow us to determine risk-associated factors in order to implement preventative actions. In the general surgery department of our university hospital we created a tool based on the registers of continuous vigilance of patient events. The objective of this paper is to describe the evolution that our tool has undergone in adapting to two purposes: logging the information in order to learn about and improve patient security, and making professionals responsible for risk and error management as another activity in their daily work. The updated version that we present is a software program which is attached to our electronic prescription system on the intranet, which was specifically designed for the Department of Surgery. As a new contribution, it presents complications in a more visual form. This allows us to have a quick understanding of the different events that each patient suffers with a color code scale which helps surgeons to know the status of each patient.
