CONFIRMAN LA IMPORTANCIA DEL DIAGNOSTICO MOLECULAR EN LA TUBERCULOSIS EXTRAPULMONAR

El diagnóstico de la tuberculosis extrapulmonar, se dificulta ya que la mayoría de los casos son paucibacilares. El cultivo que es el estándar de oro bacteriológico, casi siempre resulta negativo así como la baciloscopía que es la prueba de escrutinio más utilizada. Por lo tanto es de suma importancia validar otras pruebas diagnósticas rápidas y sensibles como la PCR.

Resumen
Introducción: La tuberculosis extrapulmonar (TBEP) representa un gran problema diagnóstico, ya que la tinción de Ziehl-Neelsen (ZN) y el cultivo son útiles, pero no pueden detectar un importante número de casos debido a que son poco bacilíferos (paucibacilares). Objetivo: Determinar la utilidad de la reacción en cadena de la polimerasa (PCR) anidada en el diagnóstico de la enfermedad. Material y métodos: De septiembre 2002 a febrero 2008 se estudiaron 469 pacientes con sospecha de TBEP. Se consideró el perfil clínico de los pacientes y se tomaron las muestras biológicas correspondientes. Después de que se procesaron las tinciones de ZN y los cultivos en Lowenstein-Jensen (LJ), se extrajo el DNA y se hizo la PCR anidada. Al final se compararon los resultados de todas las pruebas. Resultados: Se confirmaron 183 pacientes con TBEP, en base a parámetros de laboratorio (ZN, cultivo, PCR positiva), imagenología y respuesta clínica al tratamiento. Conclusión: La PCR puede considerarse como una herramienta valiosa para el diagnóstico de TBEP, a la par con ZN, cultivo, imagenología y respuesta al tratamiento.

Abstract
Introduction: Extrapulmonary tuberculosis (EPTB) represents a great diagnostic problem, since Ziehl-Neelsen (ZN) staining and culture are useful but cannot detect an important number of cases due to a paucity of bacilli on smears (paucibacillary). Objective: To determine the utility of the nested polymerase chain reaction (PCR) in the diagnosis of the disease. Material and methods: From September 2002 to February 2008, 469 patients with suspected EPTB were studied. The clinical profile of the patients was evaluated and the corresponding biological samples were obtained. After ZN staining and culture in Lowenstein-Jensen (LJ) were processed, mycobacterial DNA was isolated from specimens and the nested PCR was performed. Results of ZN, culture and PCR were compared. Results: There were 183 confirmed EPTB patients, based on laboratory parameters (either culture isolation of Mycobacterium tuberculosis and ZN or any two, positive PCR), imaging studies and clinical response to therapy. Conclusion: PCR can be considered a valuable adjunct to ZN, culture techniques, imaging studies and response to therapy in the diagnosis of EPTB.
